This type of request has three parts. (1) Please complete as many fields in the form below as possible. Required fields are marked. (2) You should then download, complete, and atttach the Request for Constitutent Services .pdf located below (Privacy Act Form). (3) Optionally, attach the Privacy Act Form along with a description detailing the situation you are experiencing with the specific Federal Agency and hit submit or you can print the generated Privacy Act Form and mail/fax it to our district office at (702) 837-0728

We must have your signature to proceed with this type of request.
